# How to Build a Lead Qualification Quiz: Scoring Template and Worked Example

A product-neutral method for turning buyer answers into a documented fit score, routing rule and measurable next step.

## The direct answer

Build a lead qualification quiz by defining the sales decision first, asking only questions that can change that decision, separating fit from readiness, assigning explicit points and disqualifiers, and mapping every score band to a useful next action. Test every boundary and contradictory answer combination before launch. A score is a routing aid, not proof that a person will buy.

Key findings

## What matters most

These conclusions define the decision boundary used throughout the guide.

- A qualification question belongs in the quiz only when its answer changes scoring, routing or follow-up.
- Fit and readiness should be recorded separately before they are combined into one priority rule.
- Boundary tests and disqualifier tests catch routing errors that a happy-path submission will miss.01

## What is a lead qualification quiz?

A lead qualification quiz is a short sequence of questions that collects volunteered information about a buyer, applies a documented rule and returns an appropriate next step. It differs from a generic lead form because the answers affect the result. It differs from a personality quiz because the result is an operational decision such as book, nurture, redirect or decline.

Start with the decision your team already makes. If sales uses company size, use case, authority, budget and timing, the quiz can collect those signals. If an answer will never change how the contact is handled, remove the question or collect it later. Typeform's current guide similarly recommends keeping a qualification form focused and suggests no more than six questions, but that is vendor guidance rather than a universal benchmark.

## Use the lead qualification quiz scoring worksheet

The worksheet below separates the business rule from the interface. Complete it before writing polished question copy.

Keep a short rationale beside every rule. HubSpot's current scoring documentation distinguishes fit scores based on record properties from engagement scores based on events. The same separation is useful here even if the final quiz uses a simpler combined priority band.

03

## Choose score bands, then define the action

Add positive points for evidence of fit or readiness. Use negative points sparingly, because they can hide contradictory answers inside a plausible total. Use an explicit disqualifier when one answer makes the offer inappropriate regardless of the score.

These numbers are an example, not a benchmark. Derive your thresholds from your own qualification policy, then compare the bands with later outcomes. A low-fit but highly engaged contact should not silently become high fit just because several activity points offset the mismatch.

04

## Worked example: score a six-answer response

Assume a respondent selects the target team (+20), 12 seats (+15), a supported use case (+20), a launch in six months (+5), budget not yet approved (+0), and recommender authority (+5). The total is 65, with no disqualifier. The routing matrix places the contact in the middle band: deliver the useful result, collect the missing readiness detail and begin a relevant education path rather than immediately presenting a high-priority booking request.

Now change only the use-case answer to an unsupported requirement. Even if every other answer earns the maximum score, the disqualifier wins. This is why disqualifiers should be modeled as rules, not large negative numbers that another set of answers can cancel.

05

## Run nine tests before publishing

- Submit the minimum possible score.
- Submit the maximum possible score.
- Test one point below and exactly on every threshold.
- Test one point above every threshold.
- Combine a high score with each disqualifier.
- Combine strong fit with low readiness.
- Combine weak fit with high readiness.
- Leave every optional answer blank once.
- Confirm that every result supplies a useful next step.

Record expected and actual results for each synthetic submission. If the platform stores contact properties or sends data elsewhere, verify that the stored values match the visible answers and that retries do not create duplicate records.

06

## Measure the decision, and know its limits

Track starts, completions, result-band distribution, next-step clicks, booked meetings and later accepted opportunities. Do not optimize completion in isolation. Removing a useful qualification question may raise completions while lowering the proportion of contacts the sales team can act on.

The model is a hypothesis. Review false positives, false negatives and unanswered questions with the team using the result. Reweight only with enough downstream evidence and keep a change log so historical scores remain interpretable. Do not use a lightweight marketing quiz as the sole basis for employment, credit, health, insurance, legal or other consequential decisions.
